肠道基石菌产丁酸菌匮乏诱发帕金森的机制研究进展

【摘要】帕金森病(PD)的发病机制与肠-脑轴密切相关,其中肠道菌群失调,特别是产丁酸菌的减少,在疾病进程中发挥重要作用。酪酸梭菌(丁酸梭菌)作为一种肠道健康的基石菌,能通过代谢产生丁酸(酪酸),在PD调控中展现多靶点神经保护潜力。本文系统总结了酪酸梭菌的生物学特性及其在PD中的菌群变化,并重点阐述了丁酸通过增强肠道屏障功能、抑制α-突触核蛋白聚集、缓解小胶质细胞介导的神经炎症、促进线粒体自噬以及调节GLP-1和脑源性神经营养因子表达等多条途径发挥保护作用的机制。然而,当前证据仍存在高质量临床试验匮乏等局限,因此未来需进一步阐明其信号网络,并推进临床转化研究。

1. 引言

帕金森病(PD)是全球第二常见的神经退行性疾病,其患病率随人口老龄化而快速上升[1]。PD的临床特征包括运动迟缓、肌强直、静止性震颤和姿势步态异常,同时伴有嗅觉减退、快速眼动睡眠行为障碍等非运动症状[2]。其核心病理改变为中脑黑质致密部多巴胺能神经元的进行性丢失,以及残存神经元内α-突触核蛋白(α-Syn)的异常聚集形成路易小体[2]。当前PD的治疗主要基于左旋多巴替代多巴胺,但这类方法仅能缓解部分症状,无法阻止疾病进展,且长期使用会诱发运动波动和异动症等严重并发症[2]。近年来,肠-脑轴在PD发病机制中的作用受到广泛关注,肠道菌群失调及其代谢产物的异常已被证实参与PD的病理过程[3]。 因此,靶向肠道菌群干预策略,有望成为干预PD进程的新方向。

Braak等人提出的病理分期假说认为,PD的病理改变可能始于延髓的迷走神经背核和嗅球,随后以向上方式逐渐扩展至中脑及皮层[3]。临床流行病学研究为这一假说提供了支持:胃肠道功能障碍会先于运动症状出现长达20年[4];完全性迷走神经干切断术与PD发病风险降低相关[5];此外,炎症性肠病(IBD)患者罹患PD的风险显著增加[6]。这些发现提示,肠道病理改变,尤其是慢性炎症,可能在PD的早期发生中发挥重要作用。

作为连接肠道与大脑的关键环节,肠道菌群在PD病理生理过程中的作用日益受到重视。肠道健康的基石菌酪酸梭菌(又称丁酸梭菌)是一种严格厌氧的革兰氏阳性产丁酸(又称酪酸)共生菌,能够形成芽孢,使其抵抗胃酸和胆汁酸并顺利到达结肠定植,在亚洲国家已有数十年的安全使用历史[7, 8]。近年来的研究表明,酪酸梭菌能通过其代谢产物丁酸,在PD中发挥神经保护作用[9]。

本文系统综述总结PD中的产丁酸菌群变化,并重点探讨酪酸梭菌通过多个途径调控PD的潜在机制,同时整合了现有动物实验和临床研究的证据。

2. PD患者肠道中的产丁酸菌及丁酸匮乏

酪酸梭菌作为重要的丁酸生产者,通过丁酸激酶途径产生丁酸,而丁酸不仅是结肠上皮细胞的主要能量来源,还能通过肠-脑轴影响中枢神经系统功能[7, 10, 11]。多项基于宏基因组及16S rRNA基因测序的Meta分析一致表明,PD患者肠道内酪酸梭菌等产丁酸菌的丰度显著降低[12-14]。Nishiwaki等人对六个国家的鸟枪法宏基因组数据进行分析后发现,PD患者粪便中产生丁酸的罗斯氏菌和普拉梭菌显著减少,同时丁酸等短链脂肪酸(SCFAs)水平也显著下降[12]。此外,针对未接受治疗的早期PD患者的研究显示,毛螺菌科及其下的罗斯氏菌属等菌群在疾病初期即已减少,这些产丁酸菌的减少并非药物所致[15]。由此可见,酪酸梭菌等产丁酸菌在PD患者中的丰度减少,导致丁酸匮乏,进而削弱肠-脑轴的多重神经保护功能。

3. 酪酸梭菌调控PD的潜在机制

酪酸梭菌主要通过其代谢产物丁酸,在调控PD的多个病理环节中发挥作用。丁酸作为一种组蛋白去乙酰化酶(HDAC)抑制剂和G蛋白偶联受体(GPCR)配体[16, 17],通过多重信号通路实现对PD的神经保护。

3.1 调节肠道菌群结构与屏障功能

酪酸梭菌能够调节紊乱的肠道微生态,增加有益菌的同时抑制条件致病菌[9]。在MPTP诱导的PD小鼠模型中,酪酸梭菌干预能恢复部分产丁酸菌的丰度[9]。同时,丁酸是维持肠道上皮屏障功能的关键分子。研究表明,PD患者存在肠道紧密连接蛋白表达降低和肠道通透性增加(即“肠漏”)[18]。丁酸通过激活AMPK信号通路,促进紧密连接蛋白的组装与表达,从而增强肠道屏障功能[19]。此外,丁酸还能刺激肠上皮细胞分泌黏蛋白MUC2,加强黏液层保护[7]。通过修复受损的肠道屏障,丁酸能有效减少细菌脂多糖(LPS)等促炎产物的易位,降低系统性炎症水平,进而减轻对中枢神经系统的炎性攻击[20]。

3.2 抑制α-Syn聚集与传播

α-Syn的异常聚集是PD的核心病理特征,而肠道屏障受损导致的“肠漏”是促使α-Syn在肠道异常聚集并可能向中枢传播的早期事件。增强肠道屏障功能、减少LPS等促炎产物的易位,能间接抑制外周炎症对肠神经系统中α-Syn的诱导[20, 21]。此外,丁酸作为一种HDAC抑制剂,能拮抗α-Syn在细胞核中抑制组蛋白乙酰化的毒性作用,从而直接减轻α-Syn介导的神经元损伤[22]。动物研究进一步证实,补充酪酸梭菌或丁酸钠能显著减少PD模型小鼠黑质和结肠中的磷酸化α-Syn聚集[9, 20, 23],从而在肠-脑轴两个层面抑制该蛋白的异常沉积与传播[24]。

3.3 调节神经炎症与小胶质细胞活化

神经炎症是PD的关键环节,小胶质细胞过度活化在其中起核心作用。病理刺激下,小胶质细胞释放TNF-α、IL-1β和iNOS等促炎介质,加剧多巴胺能神经元损伤[25, 26]。酪酸梭菌的代谢产物丁酸能有效抑制小胶质细胞活化:在PD动物模型中,丁酸处理可降低Iba-1和CD11b等活化标志物表达,并下调TNF-α、IL-1β和IL-6水平[20, 23, 27]。其机制主要涉及抑制NF-κB p65磷酸化及MAPK信号通路(p38、JNK),从而减少促炎因子转录[23, 25]。该抗炎作用具有脑区特异性,在黑质和纹状体尤为显著,与小脑等不易受累区域形成对比[28]。通过抑制小胶质细胞介导的神经炎症,丁酸可直接减轻中枢炎症损伤,并协同改善肠道屏障功能,从肠-脑轴两个层面抑制α-Syn聚集及其触发的炎症级联反应[23, 27]。

3.4 改善线粒体功能与自噬

线粒体功能障碍是PD的核心病理环节之一。研究表明,酪酸梭菌代谢产生的丁酸可通过多种途径改善线粒体质量控制和自噬功能。在MPTP诱导的PD小鼠模型中,补充丁酸钠可显著减轻线粒体氧化应激,并降低α-Syn水平[20]。机制研究发现,丁酸能够通过上调PINK1/Parkin通路促进损伤线粒体的自噬性清除,表现为LC3B-II/LC3B-I比值和Beclin-1表达升高、p62水平降低,并伴随自噬溶酶体形成增多[29]。此外,丁酸作为HDAC抑制剂,可通过表观遗传上调PGC-1α表达,进而增强自噬通量并促进α-Syn的降解[23, 29]。上述证据表明,酪酸梭菌通过代谢产生的丁酸能够通过PINK1/Parkin和PGC-1α相关通路双重改善线粒体功能、激活自噬,从而减轻多巴胺能神经元损伤。

3.5 调节神经递质与神经营养因子

丁酸还能通过肠-脑轴间接或直接调节中枢神经递质水平。首先,丁酸能通过激活肠道L细胞表面的GPR41/43受体,刺激GLP-1分泌[30]。GLP-1作为一种肠促胰岛素激素,能够穿过血脑屏障,在中脑中发挥神经营养和抗凋亡作用[31]。Sun等人的研究发现,口服酪酸梭菌能增加PD小鼠结肠GLP-1水平,并上调脑内GLP-1受体表达,伴随酪氨酸羟化酶(TH)阳性神经元增多和行为改善[9]。其次,在6-OHDA大鼠模型中,丁酸钠治疗能显著提高纹状体多巴胺水平,并上调脑源性神经营养因子(BDNF)的表达,这与其HDAC抑制活性诱导的BDNF基因转录增强有关[32]。

4. 产丁酸菌及丁酸对PD的干预效果

4.1 动物实验与细胞模型研究证据

大量动物实验支持酪酸梭菌及其代谢产物丁酸对PD的改善作用。在行为学层面,丁酸钠或酪酸梭菌灌胃能显著缩短MPTP或鱼藤酮诱导的PD小鼠在转棒、爬杆等行为学测试中的运动障碍时间[9, 23, 33]。在神经病理学层面,上述治疗能显著增加黑质区TH阳性神经元数量,减少α-Syn聚集[20, 34]。在细胞模型中,丁酸预处理能减轻MPP[+]诱导的SH-SY5Y细胞凋亡和炎症反应[26]。最近的研究进一步探索了工程化酪酸梭菌的潜力,如表达GLP-1的工程化酪酸梭菌在PD模型中展现了神经保护效果[29]。

4.2 临床研究与人源证据

当前证据仍存在高质量临床试验匮乏局限,临床试验主要集中在使用益生菌/益生元或分析PD患者体内丁酸水平。一项随机、双盲、安慰剂对照试验表明,补充含有多种菌株的益生菌能显著改善PD患者的症状[35]。此外,一项开放标签研究发现,在补充产丁酸的益生元8周后,PD患者的非运动症状和粪便钙卫蛋白(肠道炎症标志物)水平显著降低,且粪便丁酸水平升高[36]。这些临床研究为通过靶向肠道菌群提高丁酸水平以改善PD症状提供了有力的支持。

5. 研究的现状与局限性

尽管目前的研究普遍揭示了PD患者肠道中产丁酸菌匮乏和丁酸水平的降低,以及补充酪酸梭菌在动物模型中的神经保护潜力,但该领域仍存在诸多问题和挑战。当前,绝大多数证据来源于MPTP、鱼藤酮或6-OHDA诱导的急性PD动物模型,这些模型难以完全再现人类PD缓慢、渐进的病程[37, 38]。动物实验成果向临床的转化仍不充分,现有试验多为小样本、开放标签或基于混合菌株的干预,关于酪酸梭菌在PD患者中的高质量、真实世界前瞻性研究数据仍然较为匮乏[28, 39, 40]。此外,丁酸作为关键的效应分子,其在肠-脑轴中发挥作用的精确信号网络尚不完全明晰[41]。尽管如此,鉴于酪酸梭菌等产丁酸菌在肠-脑轴中的重要作用及其多靶点神经保护潜力,该领域仍展现出从调节肠道产丁酸菌入手干预PD的广阔前景。

6. 总结与展望

综上所述,肠道菌群失调,特别是酪酸梭菌等产丁酸菌的减少,在PD的发病机制中扮演着重要角色。酪酸梭菌作为一种安全高效的产丁酸益生菌,其产生的丁酸通过多靶点、多途径发挥神经保护作用,包括恢复肠道菌群平衡、增强肠道屏障功能、抑制α-Syn聚集、调节小胶质细胞介导的神经炎症、改善线粒体自噬以及调节神经递质和神经营养因子。这一以酪酸梭菌为核心的干预策略,契合了通过调控肠道微生态的关键基石菌产丁酸菌来改善中枢神经系统功能的循证医学观点,为拓展PD的防治策略提供了新的研究思路。

我国在酪酸梭菌的基础研究与产业化应用方面已取得重要进展。其中,酪酸梭菌菌株CGMCC0313-1具有自主知识产权,相关技术已获得中美两国发明专利授权(ZL 200610086642.3,US 7785581)。基于该菌株开发的酪酸梭菌活菌制剂展现出良好的安全性与应用前景,酪酸梭菌活菌胶囊与酪酸梭菌活菌散等微生态制剂已实现规模化生产,并经国家药品监督管理局批准为绿标OTC药品[42-43]。面对该领域现存的诸多问题和挑战,我国的上述研发成果为进一步阐明其作用机制及推动临床转化提供了可靠的菌株资源与制剂保障。
