肠道/健康基石菌——酪酸梭菌防治儿童过敏性疾病的作用机制

【摘要】儿童过敏性疾病已成为全球性公共卫生问题,涵盖多种表型,严重影响患儿生长发育与生活质量。传统对症治疗难以从根本上纠正免疫失衡,存在疗程长、易复发等不足。肠道菌群紊乱是过敏发生发展的关键危险因素。基石菌酪酸梭菌通过修复肠道屏障、调节肠道菌群、调控免疫、抑制炎症、调控“肠-皮轴”、“肠-肺轴”等多靶点途径发挥抗过敏作用。本文系统梳理儿童过敏性疾病的流行现状、核心机制与传统治疗局限,重点总结了酪酸梭菌在儿童过敏性疾病中的临床应用证据与作用机制,并指出我国在酪酸梭菌制剂产业化方面的先发优势,为其在儿童过敏领域的转化应用奠定基础。

一、儿童过敏性疾病的发病现状及危害
儿童过敏性疾病发病率持续攀升,已成为全球重大公共卫生挑战。美国疾病控制与预防中心(CDC)2024年全国性调查数据显示,美国0-17岁儿童中29.5%至少患有一种过敏性疾病,其中季节性过敏性鼻炎(20.6%)、特应性皮炎(12.7%)、食物过敏(5.3%)最为常见[1]。
全球范围内,20%~30%的儿童受过敏困扰,食物过敏、特应性皮炎、过敏性鼻炎和哮喘的患病率分别达5%~10%、15%~20%、10%~40%和5%~20%[2][3]。中国区域性研究亦显示,儿童青少年过敏性鼻炎患病率在过去二十年显著上升,尤其在大城市[2]。过敏不仅导致患儿年均医疗支出增加40%,还显著降低生活质量,长期未控制的过敏性哮喘可能导致肺功能受损,而食物过敏等则给家庭带来巨大的心理和经济压力[2][4]。另外,低龄化趋势明显,防治难度进一步加大。据估计,全球每年因过敏造成的经济损失高达数百亿美元且持续增长[2][5]。
二、儿童过敏性疾病的发病机制
儿童过敏的发病机制涉及遗传、环境、免疫与微生物多因素交互作用,其中肠道菌群在免疫系统发育与调节中扮演关键角色[6][7]。“卫生假说”指出,过度清洁环境使婴幼儿早期缺乏微生物接触,干扰肠道菌群正常建立,影响免疫“教育”过程[8]。生命早期是免疫系统发育的关键窗口,肠道菌群的组成与功能对免疫耐受的形成具有不可逆影响[9]。
肠道菌群失衡可导致肠道屏障受损,形成“肠漏”,促使食物抗原与细菌产物进入血液循环,激活免疫系统[10][11]。这过程中,促过敏的Th2型免疫反应过度激活,而调节性T细胞功能相对不足,诱发或加重过敏[12]。此外,肠道微生物代谢产物如短链脂肪酸可通过调节免疫细胞功能,影响远端器官(如肺、皮肤)的免疫状态,形成“肠-肺轴”“肠-皮轴”等调控网络[13]。
三、传统治疗方式及局限性
儿童过敏性疾病的传统治疗(抗组胺药、糖皮质激素、过敏原规避等)虽可缓解急性症状,但存在明显局限:首先,多为对症处理,无法从根本上阻断过敏进程[14];其次,长期使用糖皮质激素可能抑制儿童生长发育及肾上腺功能[15];再者,严格规避过敏原在现实中难以实现,反复发作导致药物依赖性与依从性差,长期管理效果不佳[16]。因此,亟需能纠正免疫失衡、阻断过敏进程且安全性高的新型手段,而靶向肠道菌群的干预策略提供了突破方向。
四、酪酸梭菌的临床应用及机制机理
近年来,国内外多个研究团队从微生态基础理论的角度,深入阐释了酪酸梭菌在多种疾病防治中重要价值。2024年赵立平团队在《Cell》杂志发表研究,提出产丁酸菌是维持宿主健康的“基石功能群”[17];2026年Bisi T. Velayudhan团队进一步指出,以酪酸梭菌为代表的产丁酸菌在维护肠屏障、调节免疫、保障整体健康中发挥关键作用[18]。
4.1 酪酸梭菌在儿童过敏性疾病中的临床应用
酪酸梭菌在儿童特应性皮炎/湿疹、支气管哮喘等各类过敏性疾病的防治中,均展现出明确的临床疗效。
特应性皮炎/湿疹:王洁等研究显示,酪酸梭菌CGMCC0313-1联合丁酸氢化可的松乳膏治疗婴儿湿疹,能显著缩短皮损、消化不良、腹泻及瘙痒等症状的缓解时间,提高治疗总有效率,并降低复发率,显示出良好的辅助治疗效果[19]。
支气管哮喘:李竹青的研究表明,在标准治疗基础上加用酪酸梭菌CGMCC0313-1,可显著改善儿童哮喘控制测试(C-ACT)评分,降低血清免疫球蛋白E(IgE)和嗜酸性粒细胞水平,并有效提升肺功能指标(第1秒用力呼气容积、用力肺活量比值、呼气峰流速),总体显示出优于单纯标准治疗的临床疗效[21]。
4.2 酪酸梭菌防治儿童过敏性疾病的作用机制
酪酸梭菌之所以能取得上述显著的临床疗效,主要源于其多靶点的作用机制,具体体现在以下几个方面:
4.2.1 构筑肠屏障
完整的肠道上皮屏障是阻止过敏原进入机体、触发过敏反应的第一道防线,酪酸梭菌可通过多途径修复过敏患儿受损的肠道屏障。
酪酸梭菌的主要代谢产物丁酸是结肠上皮细胞的首要能量来源,占其能量需求的70%以上,可促进肠上皮细胞增殖与损伤修复,维持肠道黏膜的完整性[22]。
酪酸梭菌可显著上调肠道上皮细胞紧密连接蛋白claudin-1、occludin、ZO-1的表达,增强肠上皮细胞间的紧密连接,降低肠道通透性,减少未消化的食物过敏原透过肠黏膜进入血液循环[23]。最新研究发现,酪酸梭菌可通过上调肠上皮细胞双孔钾通道Trek1的表达,改善过敏小鼠的肠道黏膜通透性,缓解肠道过敏反应,这是酪酸梭菌修复肠屏障的全新分子机制[24]。
酪酸梭菌可促进肠道杯状细胞增殖与黏蛋白分泌,提升肠道黏液层厚度,形成物理屏障,减少过敏原与肠上皮细胞的接触[25]。
4.2.2 调节肠道菌群
肠道菌群紊乱是儿童过敏性疾病发生的核心始动因素,酪酸梭菌可从多个维度重塑过敏患儿的肠道微生态平衡。其一,酪酸梭菌可通过产生丁酸等短链脂肪酸,降低肠道内pH值,选择性促进有益菌的增殖,同时抑制大肠杆菌、金黄色葡萄球菌、艰难梭菌等致病菌的生长,优化肠道菌群结构[26]。其二,酪酸梭菌可显著提升过敏患儿肠道菌群的物种多样性,恢复生命早期肠道菌群的正常定植模式,而菌群多样性提升是诱导免疫耐受形成的关键[27]。其三,酪酸梭菌作为重要的产丁酸菌,可补充过敏患儿肠道内缺失的产丁酸菌丰度,恢复肠道短链脂肪酸代谢水平,重建肠道微生态的代谢稳态,阻断菌群紊乱介导的异常免疫激活[28]。
4.2.3 调控免疫
免疫耐受缺失是过敏性疾病的核心病理基础,酪酸梭菌可通过调控固有免疫与适应性免疫的多个环节,纠正免疫失衡,重建机体对过敏原的免疫耐受,这是其发挥抗过敏作用的核心机制。
4.2.3.1 纠正Th1/Th2细胞亚群失衡
酪酸梭菌可逆转过敏患儿体内的Th2型优势免疫应答。其代谢产物丁酸通过抑制GATA3等Th2型转录因子表达,减少IL-4、IL-5、IL-13等Th2型细胞因子分泌,进而抑制B细胞产生过敏原特异性IgE,降低致敏状态。同时,酪酸梭菌可促进Th1细胞分化,上调T-bet表达,增加IFN-γ分泌,恢复Th1/Th2细胞平衡[29]。
4.2.3.2 促进Treg细胞分化与功能成熟
酪酸梭菌可强效诱导肠道Treg细胞增殖分化。其代谢产物丁酸作为组蛋白去乙酰化酶(HDAC)抑制剂,通过表观遗传调控上调Foxp3基因表达,促进初始CD4+T细胞向功能性Treg细胞分化。同时,酪酸梭菌可促进Treg细胞分泌IL-10、TGF-β等抗炎细胞因子,抑制效应T细胞过度活化,诱导免疫耐受[30]。
4.2.3.3 调控抗原呈递细胞功能
酪酸梭菌可调控树突状细胞(DC)的成熟与功能,促进耐受性DC形成,下调CD80、CD86等共刺激分子表达,减少促炎细胞因子分泌,诱导Treg细胞分化,抑制Th2型免疫应答[31][32][33]。同时,酪酸梭菌可下调DC表面TLR4表达,抑制TLR4/MyD88/NF-κB促炎信号通路激活,阻断过敏炎症的始动环节[33][34]。
4.2.4 抑制炎症
最新的前沿研究证实,酪酸梭菌可通过“肠-皮轴”、“肠-肺轴”等肠-靶器官轴,调控远端皮肤、呼吸道等过敏靶器官的炎症反应,这是其发挥全身抗过敏效应的重要机制。
在肠-皮轴调控中,酪酸梭菌可通过改善肠道菌群紊乱与肠屏障功能,减少肠道入血的促炎物质,抑制全身系统性炎症[35];同时,其代谢产物丁酸可通过血液循环到达皮肤组织,抑制皮肤角质形成细胞的异常活化与炎症因子分泌,修复皮肤屏障功能,减少皮肤朗格汉斯细胞的过敏原呈递,抑制皮肤Th2型炎症反应,缓解特应性皮炎的皮损与瘙痒症状[26][36][37]。
在肠-肺轴调控中,酪酸梭菌及其代谢产物可通过血液循环、免疫细胞迁移,调控呼吸道黏膜的免疫微环境。丁酸可通过血液循环到达肺部,抑制呼吸道上皮细胞的炎症因子分泌,修复呼吸道黏膜屏障,降低气道高反应性[38];同时,酪酸梭菌可促进肺部组织Treg细胞分化,抑制Th2型炎症与嗜酸性粒细胞浸润,缓解哮喘气道炎症与重塑,改善肺功能[39][40]。研究证实,酪酸梭菌可显著降低卵清蛋白诱导的哮喘小鼠的气道阻力、气道炎症与气道重塑,减少肺部过敏原特异性IgE表达,为其在儿童哮喘中的应用提供了机制支撑[38]。
五、总结
儿童过敏性疾病的发病率持续上升,已成为全球关注的重大公共卫生问题。其发病机制复杂,核心在于肠道菌群紊乱导致的免疫耐受缺失与异常Th2型免疫应答。传统治疗手段虽能缓解症状,但难以实现疾病修饰,亟需安全有效、可重建免疫平衡的新型干预策略。
基石菌酪酸梭菌通过产丁酸等多重机制,在修复肠道屏障、调节肠道菌群、调节免疫反应、抑制过敏炎症等方面发挥关键作用。临床研究也已证实,其在特应性皮炎、食物过敏、支气管哮喘等儿童过敏性疾病中具有明确的辅助疗效,能显著缓解症状、降低复发、改善免疫指标。
值得关注的是,我国在酪酸梭菌的产业化应用方面走在世界前列。酪酸梭菌相关制剂早在2004年便获国家药品监督管理局批准,作为绿标OTC非处方药上市,标志着我国领先欧美等国二十余年实现了产丁酸菌药品制剂的产业化。这一先发优势为我国在儿童过敏性疾病微生态治疗领域提供了坚实的临床转化基础与应用保障。
